The family doctors participation in life quality improvement of people living with HIV/AIDS (PLWHA)

Issues: Kaliningrad has become the first city of Russian Federation were the HIV infection has started in the year of 1996 among intravenous drug users. So, Kaliningrad may be considered as the indicator of HIV epidemic development in Russian federation. It is expected that in upcoming 2-3 years Kaliningrad is going to be the city, which will face the first tremendous raise of the level of AIDS in Russia. It will be the result of complex different causes, including the age of epidemic, lock of antiretroviral therapy access, etc. Existing problem of stigmatization put the border between effective medical and social care of PLWHA. Description: Necessity to use most flexible just in time medical care system for PLWHA life quality support becomes obvious. Such medical care system is based on Family medicine practices as the main part of primary care is being introduced in Kaliningrad region. General Practitioners (GP) are trained to use wide medical knowledge in patient assessment. This allows in time diagnose various symptoms of HIV/AIDS development. This is especially important in rural districts. GP must become trusted physicians who will brake stigmatization barrier. This physicians are also must provide connection between regional anti-AIDS Center and all medical institutions as well as with palliative care network. Lessons learned: During provided SWOT analysis and qualitative Ischikava cause-effect analysis among health care professionals and representatives of health care authorities it was discovered the weakness in training of physicians in HIV/AIDS related knowledge. Introducing of GP in the region may resolve important issues in PLWHA care. Recommendations: During period of introducing of GP it is important to stress out on skills building courses. The main aim of these courses is to develop knowledge of GP in the clinic, diagnosis and treatment of HIV/AIDS and associated diseases.
