HIV clinical capacity building in the US-affiliated Pacific Jurisdictions: lessons learned

Issues: Pacific AIDS education and training center (AETC) through their Hawai’i AETC partner, has engaged in an HIV clinician capacity building effort in the 6 US-affiliated Pacific Jurisdictions. Previous STD and TB clinician training assessments indicated a strong need for HIV content among regional clinicians.
Description: A longitudinal strategy was developed to build capacity for high quality HIV care, treatment and ongoing training among regional clinicians. The Pacific Basin Medical Association and the Pacific Nurses Leadership Conference were engaged as primary facilitating partners. Needs assessment, key informant interviews, hands-on clinical training, , and jurisdiction specific HIV consultation were part of the plan. Off island miniresidency experiences were developed for clinician in the relatively high HIV prevalence areas of Guam (estimated 150 HIV cases) and Saipan (estimated 50 HIV cases). Twenty core clinicians representing all 6 jurisdictions were identified and enrolled into ongoing training. Based on key informant interviews and survey, the first annual training (January 2001) focused on HIV prevention, assessment and treatment, cultural sensitivity issues in HIV populations, regional problem solving, and basic training principles. The training was highly ranked by participants. Rapport built among local and US-based clinicians has allowed expert HIV consultation to assist in several new cases.
Lessons learned: Developing trust, wide exploration of barriers to care, longterm commitment and sensitivity to cultural and geographic aspects of the region are integral parts of an HIV capacity building strategy. Continued support of the core faculty will be key to the success of this plan.
Recommendations: Future areas identified for training include laboratory specimen preparation, HIV transmission, oral and perinatal HIV care. Ongoing clinician and health leadership needs assessment must inform future phases of the capacity building project.
