POWER (positive wellness & renewal)--A prevention for positives demonstration project

In January 2001, AIDS Project Los Angeles (APLA) was awarded funding to develop a centers for disease control and prevention (CDC) demonstration project to provide primary HIV prevention services to HIV-infected individuals in Los Angeles County. APLA’s prevention for positives program, called POWER (positive wellness & renewal), provides wellness case management (WCM) and health promotion program (HPP) services to individuals living with HIV/AIDS. WCM is a combination of traditional case management and client-centered prevention counseling that provides highly individualized and intensive support, while HPP complements WCM by providing prevention support at individual and group levels and to individuals who may not need individual case management. Thus, WCM, an integration of care and prevention services, became a collaborative between two traditionally separate APLA departments - care/client services and education. Setting up an effective integration between two functionally separate divisions in order to provide comprehensive service delivery is a worthwhile process to undertake, although not without its challenges. Processes of program development, challenges, successes, and lessons learned will be discussed. Process and outcome evaluation data will also be presented.
