Demographic and clinical profile of 328 HIV seropositives of Kerala, South India: A preliminary report

Background : This study was to record the clinical profile of 328 HIV seropositive patients reported at the Medical College Hospital, Trivandrum during the period January 1998 through May 2001.
Methods: The physical examination was done at the Dept. of Internal Medicine and Oral Pathology.
Results: The study group showed a male predominance of 4:1 and the mean age was 36.84 yrs. The sources of HIV infection were through heterosexual contact (90%) followed by blood transfusion (7.5%), IV drug users (0.25%) , accidental needle prick injury (0.25%). In 6 cases the definitive source could not be determined. Analysis of their occupation showed manual labourers 131, truck drivers 82 , house wives 49 , unskilled labourers 33 , commercial sex workers 20 and in 13 the occupation was not determined. Of the 328 HIV seropositive patients 46 had no oral symptoms. The frequently associated systemic disease was pulmonary tuberculosis 101. The others were extrapulmonary tuberculosis 23 , cerebral toxoplasmosis 11, encephalopathy 17, encaphalitis 6 , pneumocystis carinii pneumonia 6, fungal meningitis 6, infections of skin and scalp 17, lower respiratory tract infections 7, venereal wart 2, molluscum contagiosum 2 and other non specific and undiagnosed infections 84. Mucosal candidiasis was the most frequent oral finding in this group (236). The other manifestations in descending order of frequencies were gingivitis 163, caries 73, pigmentation 65, periodontitis 28, ulcers 26, leukoplakia 28, angular cheilitis 22, herpes simplex 8, hairy leukoplakia 7, mulluscum contagiosum 3,leukokeratosis 2 and pericoronitis 1.
Conclusions: A disturbing observation was 15% affliction of housewives without any attributable risk behavior whereas the figure among commercial sex workers was only 6%. More than 85% of HIV seropositives have had oral symptoms. A low prevalence (2.5%) of oral hairy leukoplakia and oral / systemic Kaposi’s sarcoma recorded.
