Abstract
Equity of access to HAART in Brazil - what are the challenges ?

Issues Brasil is known to have a national AIDS programme that covers theoretically 100 % of the people who need treatment. The universality of the access top HAART is guaranteed by the constitution and operationalized through federal, state and municipal laws. Nevertheless, the dynamic of access involves limitative factors to HAART and the question of the universality meets the limits of a widespread country with extremely high levels of social, cultural and economic inequities associated with uneven medical structures to implement the AIDS Programme. The highest incidence of contamination in the poor heterosexual population shows already a highest vunlerability in relation to AIDS. Does this vulnerability appear also in the procurment for treatment with HAART?. This work is a master in public health theses that is being prepared at the national school of public health.
Description A theoretical review on the litterature on access has been made to review the litterature on access. Then a field qualitative research is the base of the study. What are the factors that ease and that difficult the access to haart ? Two municipalities in the state of Rio have been chosen to implement the field research. The study doesn´t focus one one specific vulnerable group but intends to analyze the historical difficulties through a qualitative methodology including analysis of content of the intervews.
Lessons learned Availability of medicines in health structures are not a garantee of access. All kinds of difficulties exist that prevent an easy access and these need to be reviewed. Vulnerability is bigger within the "excluded" population . Universality is a public policy goal that needs to be achieved but is not automatically achieved. Access and accessibility are relative categories.
Recommendations Laws in favor of access and free avalaibility of ARVs are necessary conditions of access to HAART in Brazil, but they are not sufficient to garantee universal access.
