ILSTIM (ANRS 082) - INTERLEUKIN 2 (IL2) ACCELERATES CD4 CELLS RECONSTITUTION IN PATIENTS WITH CD4 <200/MM3 DESPITE EFFECTIVE HAART.

Background: To evaluate the capacity of IL2 to restore immunity in patients with CD4<200/mm3 despite HAART. Methods: 72 patients with CD4<200/mm3, HIV RNA <1000 copies/ml, HAART >6 months were randomized to receive sc IL2 4.5 MIU bid in 4 cycles of 5 days, every 6 weeks (W), up to W24 in addition to their prior HAART (IL2 group) or to maintain HAART alone (control C group). After 24 weeks, IL2 was proposed to patients from both groups up to 80 weeks for a total of 11 cycles in IL2 group and 7 cycles. Results: 70 patients -34 in IL2 group, 36 in C group- were evaluated at W24 of whom 28 (82%) and 29 (80%) respectively entered IL2 non-controlled phase baseline CD4 were 142/mm3 (25-199); median time on HAART was 18 months; 60% pts had prior AIDS diagnosis. At W24, IL2 induced a significant increase in CD4 cells/mm3 (+65 in IL2 vs +18 in C); median increase in CD4 AUC was +51 vs +11 and 81% (IL2) vs 33% had CD4 >200 mm3 (p<0.0001). At week 80, median CD4 cells was 380 CD4 cells/mm3 (+247) in IL2 and 270 cells (+161 CD4) in the initial C group. Overall, in the 63 pts who received at least one IL2 cycle, the probability to reach a CD4 count >200/mm3 was 76% after 4 cycles, 79% after 6 cycles, 90% after 9 cycles. There was a significant increase in both memory and naive CD4 T cells (p<0.001). No modification of proliferative CD4 response to CMV, tuberculin or HIV was observed over 24 weeks as no significant increase in HIV RNA. There were no unexpected adverse events. Conclusions: In patients with persistent low CD4 count <200/mm3 despite HAART, therefore at high risk of HIV disease progression, IL2 allows to accelerate significantly CD4 reconstitution. In these highly motivated patients IL2 appears reasonably tolerated with 80% of them being CD4>200/mm3 after 6 cycles.
