Family oriented integrated management of paediatric HIV in urban no profit hospital set up in urban India

Background: To study the benefits and cost effectiveness of family oriented management in treatment of paediatric HIV patients. Families of peri-natal and paediatric patients diagnosed as sero-positive were managed by a team using integrated intervention model. Counselling, medical treatment, social & economic interventions, education, prevention and anticipatory guidance were part of this package.
Methods: A team of paediatrician, gynaecologist, physician, medical social worker and accountant managed family units. Families were given comprehensive counselling and anticipatory guidance. They were offered diagnostic, medical, preventive and anti retroviral therapy. Economic implications and possible assistance were discussed. Social acceptance and rehabilitation was emphasised in this model.
Regular follow up was ensured by the social worker. Cost effective analysis was done and compared to a situation if these cases were managed separately on individual basis.
Results: All 23 neonates delivered to sero-positive mothers were offered interventions negative and symptom free on follow up. Parents were offered appropriate treatment. These families managed their babies very well in spite of some parental deaths and illness. Families of all 47 paediatric HIV cases admitted in the hospital were managed in the same package. There was substantial reduction in hospital visits (31%), cost of treatment (45%) and a good follow up (95%) due to this intervention. Social and economic package was offered to family as a whole which resulted in much better treatment compliance (91%). Families coped up with the situation much better together than they would have done individually.
Conclusions: This model supports the concept of family oriented integrated approach in management of paediatric HIV – a disease which affects the entire family. This approach has medical, economic and social benefits and can be replicated any where in the world particularly in developing countries.
